An integrated case management approach is adopted for the support of the clients in need. A case manager will be assigned to each case registered in I-WELL to ensure interdisciplinary care coordination and successful transitions between different professionals. The I-WELL management model will go through the procedures of case intake, screening assessments, clinical assessments if necessary (speech pathology, occupational therapy and psychological assessments), problem identification, treatment implementation and coordination, consultation with interdisciplinary help and health professions, negotiation with clients’ families, family and parenting education tailor-made for clients’ families and case termination management. Case managers play an important role in helping clients move through the continuum. Data on training and progress are input into the data bank for systematic analysis. Evidence-based practice and collaboration on research are facilitated.
